PUBLIC ANNOUNCEMENT CONCERNING

WEBSTER MANOR REHABILITATION & HEALTH CARE CENTER

Webster Manor SNF Operations, LLC, 82 Wendell Ave. Ste. 100, Pittsfield, MA 01201, (the “Buyer”) has submitted an application to the Department of Public Health, Division of Health Care Facility Licensure and Certification (the “Department”) for a proposed change in ownership of Webster Manor Rehabilitation & Health Care Center, 745 Main Street, Webster, MA  01570 (the “Facility”).  The Buyer, if deemed suitable by the Department, intends to purchase the Facility from the current owner, Webster MA SNF LLC, 745 School Street, Webster, MA 01570 (the “Seller”).  There will be no changes in the bed capacity or the services provided by the Facility. A public hearing may be requested upon petition by any group of ten adults.  Such petition shall include the name, address and signature of each adult and designate one member as the representative of the group. Written comments concerning the Buyer’s ability to provide quality long-term care services and petitions for a public hearing may be addressed to the Department of Public Health, Division of Health Care Facility Licensure and Certification via email at HFLLicenseAction@state.ma.us or sent via mail to 67 Forest Street, Marlborough, MA 01752 for a period of 14 days following this publication.  All written or oral comments submitted to the Department may be posted on the Department’s website and released in response to a request for public records.
